Photo & Video Release Form – Organizer & Volunteer Instructions
All organizers and volunteers for the Fourth World Congress of Existential Therapy (June 1–7) are required to complete a Photo & Video Release Form prior to the event.
Please complete and upload your form using the steps below.
Instructions
1. Download the Form
- Download the Photo & Video Release Form (PDF) from the provided link: PHOTO RELEASE FORM
- Save it to your device
2. Open the PDF
- Open the file using Adobe Acrobat Reader
- If needed, you can download* and install it (free) from Adobe at this link here.
* If you have a MAC, go to the same link, and you will see a "more download options" in gray, underneath the "Download Acrobat Reader" button, and then choose which version.
3. Complete and Sign
- Select “Fill & Sign” in Acrobat Reader
- Enter your information
- Add your signature using:
- Place the signature in the designated field
4. Save the File
- Save the completed document as a PDF
- Recommended format: LastName_FirstName_WCET_PhotoRelease.pdf
5. Upload the Form
- Upload the signed PDF at the bottom of this page.
- Confirm the correct file was uploaded
